Redemption Instructions:

• Visit midmark.com and select “Products” then “Promotions”
• Select the 2013 Ritter Sterilizer Promotion icon
• Upload invoice*
• Complete the information required on the form
• Select “Terms and Conditions” and “Submit” All serial numbers must be submitted online. Upon each submission a confirmation email will be sent for your records. Please keep the confirmation until incentives are received. Incentives must be submitted online by October 31, 2013.

* Midmark must receive a copy of the invoice (stating the physician’s name and date purchased). Physician or Medical Facility invoice must be dated June 1 – September 30, 2013. Incentives will not be processed until the invoice is received.

Direct Relief Donation Procedure:

Within three (3) weeks upon submission of the online incentive form, Midmark will send you a prepaid FedEx® Ground label to ship the donation sterilizer. Upon receiving the prepaid FedEx® Ground label, you must package the donation sterilizer and call 1-800-GoFedEx for pickup. Direct Relief® will provide a donation receipt. Sterilizers must be received at Direct Relief® by November 30, 2013, to receive the additional $200 rebate check from Midmark – no exceptions. *NO RETURN Policy: Direct Relief® is unable to return the donation sterilizers once they are received at their warehouse location.
